Population health management (PHM) is the aggregation of patient data via business intelligence tools across multiple health information technology resources and data analysis into a single, actionable patient record. PHM seeks to improve the health outcomes of a group of people. Population health or pop health represents the health outcomes of a group of individuals. These groups can be nations, communities (geographic populations), disabled persons, ethnic groups, or any other defined group. Population health not only describes the overall health of a population but also involves the distribution of health.
