Medigy: Complex Chronic Care Management (CCM)

Complex chronic care management services are provided to those people who suffer from chronic conditions (two or more) that are expected to last 12 months at least or until the death of the patient. These services require moderate to high complexity decision making. These services include 60 minutes of clinical staff time that is directed by either a physician or qualified health care professional.
