Population health management or PHM is a concentrated holistic approach towards improving a group of individuals' patient health outcomes. Such individuals may belong to a larger group (consisting of people within a predetermined health system) or a specific geographic area and share some defined characteristics. PHM programs aggregate patient health data via business intelligence tools and provide a comprehensive clinical picture of each patient.
