Continuum of care is the term used to describe the continuity of medical care delivered to patients when they have to switch between caregivers or care institutions. Care continuum is more critical for patients who are entirely dependent on healthcare services such as patients suffering from dangerous and chronic conditions, elderly patients, and mentally vulnerable patients. Healthcare continuum has many advantages for patients. It improves the health of patients, increases the satisfaction level, and reduces healthcare costs. Chronic Care Continuum or C3 provides disease-specific educational content to the patients in their environment. C3 suggests real-life activities such as treatment adherence, nutritional awareness, social skills, and self-monitoring for disease complications, etc. All these skills help them in managing their diseases by themselves without requiring much support.
