Desert Senita Community Health Center, based in Arizona, has partnered with Clear Arch Health to implement remote patient monitoring (RPM) services. This initiative aims to address disparities in virtual care access, particularly among vulnerable populations. RPM technology enables physicians to monitor patients with chronic diseases like cardiovascular disease and diabetes remotely, providing personalized care. The FQHC offers RPM services at no cost to patients, billing insurance plans for associated case management. Additionally, the center has deployed community health workers alongside RPM services to ensure patients receive adequate support.
