Population health programs are challenging because patient populations are diverse, and patient data is spread across a wide variety of sources and locations.  At the core of any population health effort is the need for data that helps clinicians to first assess a patient’s risk and second connect with that patient in a meaningful way that improves their health.For a more complete picture, organizations also contract with health IT vendors to get dispense history from pharmacies.
